Due to the "Social Distancing" guidelines for the coronavirus, we are doing much of the normal match "check-in" work beforehand.

Check-in will be very brief-- you will bring the WAIVER and MEDICAL SURVEY forms to checkin (completed). If you still owe money on your registration, bring CASH in an envelope labeled with your shooter names.

YOU HAVE THE FOLLOWING ASSIGNMENTS

1. If you have ANY symptoms of COVID19 including but not limited to an elevated temperature or cough, etc, DO NOT ATTEND. If you feel like you are at elevated risk for COVID19, DO NOT ATTEND.

2. DOWNLOAD, PRINT, READ, AND SIGN OUR WAIVER. YOU MUST PRINT YOUR NAME LEGIBLY AT THE TOP.
YOU MUST SIGN AND DATE AT THE BOTTOM. DOWNLOAD THE PDF FILE HERE: CD 2020 WAIVER (PDF).
BRING THIS COMPLETED WAIVER TO THE MATCH.

3. DOWNLOAD, PRINT, AND BRING OUR COVID19 SCREENING QUESTIONAIRE.

BRING THIS WAIVER TO THE MATCH AND COMPLETE IT ON-SITE.
